Prescription Medication

Prescription medication may be given during the school day. All prescription medications must be in the original bottle, along with signed parental permission and a doctor's order. All permission expires at the end of the school year. New forms must be completed every year.

Once the forms are completed, the medication must be brought to school by an adult in the original container.

Self-carry Medications

If a parent and doctor allow a student to self carry a medication, signed permission must be on file in the nurses' office. Common self carry medications are listed below:

Inhalers

May be carried by the student once the permission form is in the student's file. I would prefer a back up inhaler in my office. Student must have their inhalers in order to go any field trip.

Epi-Pens

may be carried by the student once the permission form is in the student's file, but a backup epipen is required to be stored in the nurse's office.
